The following documentation is required:

1. A copy of the child's Birth Certificate
   
   
2. A copy of the child's Social Security Card
   
   
3. A copy of one of the following documents to use for Proof of Residency, which includes the issuer's name/official logo and address, a recent date, and the custodial parent's name and address:
   
   
   1. A recent PECO or PGW bill
   2. A Pennsylvania Vehicle Registration Card
   3. A recent pay stub
